The tool is available for use on the Web and on PDA platforms, and provides clinical recommendations across the continuum of care. Delivery of the application is tethered to a front-end CME training program which helps learners recognize clinical risk factors and apply the tool in practice to improve patient outcomes. Additionally, the instrument allows the learner to access back-end education so they can get more information based upon their individual needs, in a self-directed manner.
